Are you a vendor interested in purchasing a booth at our upcoming Holistic Health Fair? Great! This form will allow you to apply as a vendor and purchase your booth. If you are not approved as a vendor, you will be refunded immediately upon notice of rejection.

By purchasing a vendor booth, also known as exhibitor booth, you agree to all the vendor terms and conditions outlined here.

It is your responsibility to understand the vendor terms fully prior to purchasing a booth.

Please Note: Acceptance to participate in any of our events is contingent upon approval. Vendor must provide completed application AND payment in order to be considered for approval.

Once approved, NO REFUNDS ARE GIVEN. Only 3 people per booth maximum allowed.

The State of Wisconsin requires all event promoters (Holistic Health Fair included) to submit vendor registration information for every event hosted in the state. The information below is required, will be held securely, and provided ONLY to the State of Wisconsin as per law.

Even if you are a vendor from out of state, you are still required to submit your state's sales tax account number, EIN or social security number. Failure to provide this information can result in state fines and a rejection of vendor registration.

Provide ONLY 1 of the following: your state sales tax number, Federal EIN Number, or Social Security Number.

Booth Options & Pricing

This is a "table expo" event, meaning you do not receive a "convention center expo" size or style booth. Large booth structures will not fit, you would need to purchase 2 booth spaces to fit a larger display. Tables are spaced 18" from each other allowing vendors to enter and exit from either side of their table. Items and signage cannot impede this 18" space. No signage can be taped or hung from the walls.

With that said, there are 3 booth sizes/types:

Interior Booth:

The Interior booth is found within the inner space of the room, NOT against a wall. Booth dimensions are approximately 7.5' wide x 6' deep. This booth comes with a 6' x 2.5' table, 2 chairs and tablecloth. This booth does not come with electrical access.

Perimeter Booth:

This booth is found along the wall of the room and includes electrical access. Booth dimensions are approximately 7.5' wide x 6' deep. This booth comes with a 6' x 2.5' table, 2 chairs, electricity, and tablecloth. Must bring your own electrical cords and power strip (outlets are shared).

MAKE SURE you choose a booth location preference that matches with your booth type above. If you choose a Perimeter Booth, you must choose a booth number found along the wall.

Check this box if you will NOT NEED a table or if you are bringing your own. If you bring your own table or have a set up that doesn't include a table (massage table, chairs, etc...), your booth space cannot exceed 6' long and 2.5" wide. FYI a massage table AND a 6' table WILL NOT FIT.

Vendor Agreements

I hereby give consent for photos and/or video of any likeness of me to be used in print and online for purposes of promoting the event currently and for the future.

I hereby give consent to the Holistic Health Fair personnel to email me with information and updates related to the Holistic Health Fair.

I agree to and understand I am responsible for promoting the event and my presence to the best of my ability through newsletters, social media, web links, word of mouth, flyer distribution, etc.

I understand there are NO REFUNDS.

I understand the Holistic Health Fair will be following the venue's policies regarding Covid 19 protocols and guidelines.

I understand that nothing can be taped or adhered to the walls including signage. All banners and signs must be free standing and cannot interfere or be placed within the aisles, hallways, or other vendors’ space. I understand that small signage may be able to fit in my booth space, but large signage will not.

I understand that no candles, sage, palo santo, incense, or other smoking device can be lit or otherwise burned or set afire. I understand that if I use an essential oil diffuser; it must not cause any discomfort/distress to the people/vendors around me and if it does, I agree to stop using it immediately.

I understand there are only 3 people (vendors) allowed per booth space. If I need additional people to assist me in my booth, I understand that I must purchase 2 booth spaces to accommodate more than 3 people.

I understand that I am not allowed to produce noise via music, drumming, rattles, verbal, or otherwise that could interfere with other vendors. I understand I am being asked to keep all sounds within my booth space and be respectful of my surrounding vendor neighbors.

I understand only 1 MLM company representative may be a vendor at any event at a time represented and I must indicate on my business description that I am representing a MLM company, listing which company I represent clearly. If I am found to represent a MLM company and I did not indicate this within my company description, I will not be accepted to exhibit at any future events.

I understand only booths along the wall have electrical access and I will need to furnish my own cords and power strip. Booths in the middle of the room DO NOT have electrical access and absolutely no cords can be placed across the aisle. I understand that I may use a generator or other device that does not require a plug in outlet so long as that generator does not cause a noise nuisance or trip hazard.

I understand this is a TABLE sized vendor/exhibitor space, not a “convention-sized” expo booth space. There is not pipe and drape, or space for “large expo” booth structures. Each booth space is approximately 6’ deep allowing just enough space for 2 chairs and a 2.5’ deep table. It’s also approximately 6 feet in length with 1.5’ of empty space on either side of the table allowing vendors to enter and exit. I understand that if I bring a massage table or massage chair, A TABLE WILL NOT FIT and will not be provided. I understand that if I bring my own table, it cannot exceed the same 6’ x 2.5’ dimensions and cannot exceed my allotted booth space.

I understand that I am required to be set up by 9:45am and that I cannot begin taking down my booth until after 5:01pm. I understand that if I am not at the venue, actively setting up by 9:45am, my booth space will be forfeited and I will NOT receive a refund.

I understand that the Holistic Health Fair reserves the right to determine the eligibility of any product, company, service and/or demonstration and can approve or deny any vendor registration at their sole discretion without any cause.

I understand that I am solely liable for personal injury, or lost/stolen/damaged goods. I am financially liable for any damage caused to the hotel/venue property. I am responsible for complying with all city/state resale, licensing, and health department requirements. I am responsible for the safe use of all products and services and if providing samples or testers, I am required to have the liability insurance to cover any claims that could arise from the use of such samples or testers by attendees or other vendors.
