Vendor Application Form

** We strongly suggest you complete this form on a desktop computer and not on a mobile device.** 

Are you a vendor interested in purchasing a booth at our upcoming Holistic Health Fair? Great! This form will allow you to apply as a vendor and purchase your booth. If you are not approved as a vendor, you will be refunded immediately upon notice of rejection.

By purchasing a vendor booth, also known as exhibitor booth, you agree to all the vendor terms and conditions outlined here


It is your responsibility to understand the vendor terms fully prior to purchasing a booth.

Please Note: Acceptance to participate in any of our events is contingent upon approval. Vendor must provide completed application AND payment in order to be considered for approval.  

Once approved, NO REFUNDS ARE GIVEN. Only 3 people per booth maximum allowed.

The State of Wisconsin requires all event promoters (Holistic Health Fair included) to submit vendor registration information for every event hosted in the state. The information below is required, will be held securely, and provided ONLY to the State of Wisconsin as per law.


Even if you are a vendor from out of state, you are still required to submit your state's sales tax account number, EIN or social security number. Failure to provide this information can result in state fines and a rejection of vendor registration.

Booth Options & Pricing

This is a "table expo" event, meaning you do not receive a "convention center expo" size or style booth. Large booth structures will not fit, you would need to purchase 2 booth spaces to fit a larger display. Tables are spaced 18" from each other allowing vendors to enter and exit from either side of their table. Items and signage cannot impede this 18" space. No signage can be taped or hung from the walls.

With that said, there are 3 booth sizes/types: 

Interior Booth:  

The Interior booth is found within the inner space of the room, NOT against a wall. Booth dimensions are approximately 7.5' wide x 6' deep. This booth comes with a 6' x 2.5' table, 2 chairs and tablecloth. This booth does not come with electrical access.

Perimeter Booth:  

This booth is found along the wall of the room and includes electrical access. Booth dimensions are approximately 7.5' wide x 6' deep. This booth comes with a 6' x 2.5' table, 2 chairs, electricity, and tablecloth. Must bring your own electrical cords and power strip (outlets are shared).

MAKE SURE you choose a booth location preference that matches with your booth type above. If you choose a Perimeter Booth, you must choose a booth number found along the wall.
