New Process For Securing Your Booth

Please note there is a new process for applying and purchasing your booth! Submitting your application below DOES NOT secure your booth. Purchasing your booth in Step 2 secures your booth. We will not consider you as a vendor or review your application until the booth has been purchased. If your application is denied for whatever reason, we will immediately refund your booth purchase.

By submitting this application, and purchasing a vendor booth, also known as exhibitor booth, you agree to and are responsible for knowing, all the vendor terms and conditions outlined here. If you do not understand any of these terms or conditions, contact us for clarity.

ONLY 1 VENDOR BUSINESS PER BOOTH (2 people maximum at booth) - NO SHARED BOOTHS.

Please Note: All readers, psychics, intuitives, mediums, and the like, must schedule a 10 minute sample reading with Debra, the event organizer, prior to approval to ensure a quality, authentic experience for all our attendees. Once you complete the application and pay for your booth, please email info@holistichealthfair.org with the request to schedule the sample reading within 7 days of application submission.

Acceptance to participate in any of our events is contingent upon approval. Vendor must provide completed application AND payment in order to be considered for approval. Once approved, NO REFUNDS ARE GIVEN.

Note: If you are not at the event by 9:30am your booth will be forfeited and no refunds will be given. NO EXCEPTIONS. It doesn't matter if it only takes you 5 minutes to set up or if you got stuck in traffic. If you are not in your booth by 9:30am, you will not get a booth, PERIOD.

Company Information - Private

The State of Wisconsin requires all event promoters (Holistic Health Fair included) to submit vendor registration information for every event hosted in the state. The information below is required, will be held securely, and provided ONLY to the State of Wisconsin as per law.

Even if you are a vendor from out of state, you are still required to submit your state's sales tax account number, EIN or social security number. Failure to provide this information can result in state fines and a rejection of vendor registration.

Provide ONLY 1 of the following: your state sales tax number, Federal EIN Number, or Social Security Number.

ONLY IF YOU ARE AN EXEMPT VENDOR, Select from the options below indicating WHY you are exempt.

1 - Exempt sales only or display only

2 - Multi-level marketing company pays sales tax

3 - Nonprofit occasional sales exemption

4 - Exempt occasional sales

Check this box if you will NOT NEED a table or if you are bringing your own. If you bring your own table or have a set up that doesn't include a table (massage table, chairs, etc...), your booth space cannot exceed 6' long and 2.5" wide. FYI a massage table AND a 6' table WILL NOT FIT.

Vendor Agreements

I hereby give consent for photos and/or video of any likeness of me to be used in print and online for purposes of promoting the event currently and for the future.

I agree to and understand I am responsible for promoting the event and my presence to the best of my ability through newsletters, social media, web links, word of mouth, flyer distribution, etc.

I understand there are NO REFUNDS.

I understand that the Holistic Health Fair reserves the right to determine the eligibility of any product, company, service and/or demonstration and can approve or deny any vendor registration at their sole discretion without any cause.
